What is LFD?
✔ Online eLearning Module: Explore lessons from historic mining disasters to help identify early warning signs and learn from failures.
✔ Face-to-face workshop: A practical, one-day deep dive into HRO principles and practical skills you can take away to improve how you and your organisation learn from failures.
LFD is a mandatory course for statutory ticket holders in Queensland mines and quarries, as required by the Board of Examiners. But it’s more than a compliance requirement—it’s a game-changer for emerging leaders and supervisors who want to build a stronger, safer workplace, so all are welcome to complete LFD. Please note – the Online e-Learning activities must be completed prior to attending the face to face workshop: Learning From Disasters – Simtars
